Model and Photo Release Form

A Model & Photo Release Form is an important document that needs to be completed before a photo session and can be completed online. This form grants permission for the photographer to use the images of the model for commercial use. It also states that the model has the right to use the photos for their own personal use. The form must be signed and dated by both parties before the session and retained for future reference. By signing the Model & Photo Release Form, the model is agreeing to all the terms and conditions set out in the form. It is important to read and understand the contents of the form before signing it.

Securing Your Reservation 

Studio hours are by appointment.  Check the site calendar for available time slots and you can contact me at info@crescentmoonphoto.com to lock in your session. To secure your session, half will be collected at the time of booking via PayPal, Venmo or Zelle.

 

Your session payment is due in full at the time of your session. Your booking deposit is 100% non-refundable. Details will be provided after initial contact. See “the Process” for additional information. Colorado sales tax is applicable for all in state orders.

Studio Policies

Your session may be rescheduled once unless weather or illness is a factor, then we will work around these factors. Please reschedule dates within 30 days of original scheduled appointment.  Prices are guaranteed for 60 days from the date of your session. Prices are subject to change at any time.
 
All images included in each plan will include a natural edit of blemishes, softening of lines, dark circles under eyes, shine, color and density. All editing decisions will be discussed during your session. My goal is for your portrait to reflect your best self and say what you want to say to your audience. 
 
All images are the property of Crescent Moon Photography and are copyright protected. We agree that you may use your images on your social media sites, for modeling/acting headshots and if you use the photos for commercial gain additional fees may apply.  We ask that you give us a photo credit when posting our photos.  @CrescentMoonPhoto.  I may use your photos to promote Crescent Moon Photography on my website or social media.

What is Copyright?

A copyright is a legal device that gives the creator of a literary, artistic, musical, or other creative work the sole right to publish and sell that work. Copyright owners have the right to control the production of their work, including the right to receive payment for that reproduction. An author/artist may grant or sell those rights to others, including publishers or recording companies. Violation of a copyright is called infringement.
